Is there somewhere I can purchase 4 inserts for these? Why does the one with inserts lacking the glass bag?

I haven't had any issues with needing inserts. In the beginning they don't hold a form until you start putting items in them.

I do not have this problem. Make sure the bag is unfolded and the bottom is open and flat. Then, using the velcro on each side of the bag, Vecro each bag to its companion bag (as in the product illustration). If you ensure that the labels all face the same direction, the velcro is easy. With the bags attached to each other, they stand easy for filling.

could I wash these in washing machine?

I suggest just wiping them out with dish soap and a sponge , then rinse in water. A washing machine may ruin the stability that makes these bags stand upright.

Could you fit a standard brown paper grocery bag in these?

Perhaps, I never tried. They work perfectly just as they are. I only put clean items in them, and then take which ever bag is full to the transfer station. I did put a new label on one because I put my paper in something else.
