Q: Is this a complete set of bearings, there is only 1 main bearing in picture and 4 Rod Bearings?

Theses are main cap bearings only. There are no rod bearings they are separate from mains... Hope this helps

Q: Will these work on a 2002 lq4 6.0 cast iron block?

Yes they should fit per the application for the 6.0 LQ4. Same crankshaft only the bore is different from the 5.3.
