Very comfortable once on however it takes getting used to how to put it on due to many different straps . I have bad sciatica issues causing me to lean forward during flare ups and It has helped me to keep my posture straight. I have had a couple of posture correctors before but this one is by far my favorite. The part that goes around your shoulder and under your armpit is very comfortable and does not hurt at all! I love that the adjustments are easy to use and the Velcro works really good. I purchased the small. I am about 120 and petite so the small fits just right. I would say if you are on the fence about the size then go up one size since you can snug it up if needed.
