Could the phone fall off? Specifically the magnets separating? My concern is mainly if I was shooting a roller from the bed of a moving truck

With just vibration? No. but if you hit the phone hard enough against something, or if someone knocks into your phone then it can absolutely fall off. It happened to me and scuffed my screen (despite my case). You just have to take care with getting too close to things.

Is this model compatible with MagSafe on the new iPhone 12?

It will not work directly with MagSafe, as the magnets on the OM 4 are a much smaller ring than MagSafe. However, there is a very nice solution that works perfectly and is rock solid. All you need to do is add a MagSafe base you can get here: https://www.amazon.com/gp/product/B08QZFTWYR/ref=ox_sc_saved_title_2?smid=AF24J47XDV6ZJ&psc=1 Once you have this, place the magnet provided by DJI in the center of the ring opposite the side with the magnets. It has an indentation on the base just outside where you put the DJI magnet. Once you have this, note the top of the DJI magnet and make a small mark so you can identify the top easily. Then attach the magnet to the OM 4 and viola! You now have a MagSafe gimbal holder! Just make sure that the top of your phone lines up witht he mark you made to identify the top of the magnet.
