Will this work on a small mattress 7 inches thick?

It will, and will be almost flush with the top of mattress to top of padding. But let’s keep in mind that 7” may not be small to some.

Would this work for someone who is a stomach sleeper, desiring to sleep facedown all night so that their head is straight and not kinked to the side?

I would say no. This was purchased for my son who had retina surgery and had to be in a facedown position for 24 hours a day for over 2 weeks.

my mattress is 12 inches thick,will this fit?

I had a thick pillow top mattress and this did not fit...I had to return it.

What is the distance from the bottom of mattress to top of the device where the face pad sits?

This didn't work for me. The device is adjustable up or down.
