Received my mould and wondering if 4 holes in the bottom are adequate or if I should add some on the side?

No, the four bottom holes weren’t adequate for semi-hard cheese that require heavier weights to press. I did have to bore a few more holes. There’s another mold, same size, a tad more expensive, but had holes on the sides that helped to drain whey faster.

Where was this manufactured?

Mold is made in Ukraine, according to the packaging. It's very solidly thick, capable of withstanding 75-100 lbs (hard cheese pressing). It produced a gorgeously shaped cheese.
