Has anyone tested this with an Otterbox defender series case with the Pixel 6?

Yes, it works great together. I tried a few of the disc style chargers you just lay your phone on and they either heat up too much or don't line up at all resulting in super slow charges. This charger works, my phone prompted me to pair with it day 1. I have used my Pixel 6 with an OtterBox defender pro series case with it for over a month. It does occasionally say to line the phone up for a better connection even though it was dead center, even without moving it charged fine. The fan is wonderful keeping the unit cool and your battery safe. Adaptive charging is amazing. I liked this phone, case, and charger so much I bought them for a family member who lost their phone.

Is the wall adapter the same as the standalone google 30w charger? could i charge my pixel 7 directly from the wall adapter that comes with the stand?

They both look identical, and come with the same looking USB c cables. Reading specs it says the pixel phone charger is 30 watts, and the stand is 20 watts. They both charge my pixel 7 about the same speed. I guess it would work if you swapped the charging cube. The stand has a cooling fan. I tried the wall adaptor that came with the stand and connected to phone with the charging cable, and it worked fine.

Does the fan run all the time or only when it senses a load?

No, but it usually runs a moment after detecting my pixel 6pro. It is not loud, but if it's on your night stand and you have a quiet room, you will be able to hear it. It's not loud but it's there.
