Does this come with one or two guards?

It only comes with 1 guard. I wish they gave you an option and discount for 2. They work great for the thumb they are on. I am going to have to purchase a 2nd.

Does it come with 2 ( one for each thumb) or is it just one ?

I wish it came with two. It comes with only one, but it fits both right or left. My daughter is 3 years old. She completely destroyed the silicone band.

How do you stop a 19 month old from sucking his thumb without a "gadget?"

Gadgets didn't work for my child. If I had it to do over I would've let him suck a pacifier simply because you can take it away gradually. The thumb/finger is always in arms reach!!
