Can I use this tool with a 18 inch cake?

No, the tool is not that large. Wilton says another levels that is more like a "saw" and it is large enough for a 18 inch, probably.

Can it be used on cakes smaller than 10 inches?

Yes it can. As long as the cake is smaller than the width of the leveler it will work just fine. The height is also adjustable
