where does the ice pack go? in the top pocket? and that can also be extended for tall items but then you won't have a place for the ice pack?

This is a great question- There is no dedicated spot (pocket, etc.) that is intended for the ice pack. Instead, it can be packed amongst the contents of the lunch bag. The opening in the top compartment can be used to help pack taller items, can be used to separate items in the lunch bag to help protect items you don’t want to be crushed by others, and also helps to keep certain items away from having direct contact with the ice pack.
