I loved that its dumbed down to understand the basics and explains how you have leeway to add certain parts of medical history in another section so as long as covered and your not justifies why you put it in that section - essentially allowed me to keep my notes more accurate, simple, and to the point vs having tons of sections that redundant. VERY GOOD BUY!

...I was looking for when I made this purchase. I love the concept but I was hoping for slightly more detailed notes that indicated more areas that should be addressed in notes. Not that these are inappropriate, and not that it is the author's problem. This is an excellent book for someone new to documenting as a provider.

This book is full of basic points to cover for specific assessments and diagnosis. It's a good place to start if you are a brand new provider. I wanted something to help streamline my charting. I found the index to be less than helpful as the key words are arranged by body system or specialty. It requires way too much time to search through multiple categories until I find what I'm looking for. Would be much more useful it the index was just alphabetical. Lot's of good templates, just takes too long to find specific one. Wish I had been able to see a sample prior to ordering.

There is no dislike. This has helped me out with what to write and this even come in handy for my Dr I scribe for with the codes.

I bought it thinking it would help with Epic charting and tasks, but it has nothing useful in it. They really should provide a sample page or two so you can see what’s actually in it before wasting your money.

Immensely helpful for concise medical charting. I have used this text to develop 100’s of my own templates for documenting patient care. I am so grateful to the author as I would be somewhat lost in our EMR without this book! I have no doubt that the time spent with this book will save me hours every day in tedious charting.

I found it too basic and not really useful for medical documentation.
