How long does a 12 oz bottle last cleaning lenses once a day?

The reviewer who wrote that it takes 2.5 mL of solution to the fill line is wrong. 2.5 mL is 1/2 a teaspoon. Just eyeballing the volume should tell you that it is more than 1/2 a teaspoon. I am measuring the volume at 10 mL to the fill line. This means that each 12 oz (355 mL) bottle should last you about 30 days. This correlates to my own usage, more or less, depending on if you use the solution to rinse the container and how precise you are when filling it.

Does the box include 1 or 2 lens cases?

I have used this solution for about 15 years and the twin 12 Fl Oz pack has only ever come with one lens case included. The lens case is designed to last for two 12 Fl Oz bottles of solution. If I have ever felt I needed a new lens case before I ran out of cleaning solution, I have purchased a travel size bottle of solution and used the new lens case from that package. Every package of cleaning solution comes with one lens case, regardless of the size of bottle of cleaning solution.
