Yes it's unisex my great nieces wear their's all the time.

Do the onesies run small?

I don't think they run small. I didn't keep them because I kept getting duplicate designs so I returned but I believe they were true to size

Do they shrink substantially when you wash/dry them?

No they don't really shrink or lose color
