If im 52dd what is that compate yo 3xor 4x

I wear 44C. Following their size chart I purchased 2X. I would say it's a size too small, at least. Their size chart recommends 5X for 52DD. I would go for 6X, or even add one more size up, depending on how comfortable you want your fit. I probably won't buy this one again. Not sure but very comfy, easy to close & open if you have 2 good hands. Wash on cold machine and hang to dry. Hope this helps.

How many hooks are there? Good for arthritic, not very dexterous hands?

I believe there are six hooks. About as good as any are for bad hands.

Is a 6x big enough fo a size 50c bra 220 pounds

Possibly it is. If you know how to make a strap adjustment: the bra needs adjustments for some people. For me it is a minor adjustment folding the straps over in the back to bring the bra cups up higher in the front. I've done 7 bras already in the same brand and color. I'm not in the business of fixing other people's bras though, I just fix my own.
