Q: Mine is much louder than a ceiling fan, is there anything wrong??

These are a cheaply made but efficient exhaust fan. Mine was used as a exhaust fan in my garage for fumes or just working out there. It is loud and is ok for me as a shop fan. But I wouldn't suggest using it as a interior fan for everyday house hold living.

Q: How do you turn on the one without rope switch?

Its on as soon as you plug it in, I just replaced the receptacle (outlet) and installed the receptacle with built in light switch switch.

Q: where does the exhaust go? into the inside of wall cavity? i need it for kitchen.

I put mine in a plexi-glass panel in my bathroom window to vent shower steam. It could go into a wall for kitchen use with an exhaust boot on the exterior. For kitchens (and bathrooms for that matter), I would recommend a fan with higher CFM. This one did ok in the bath, but not great. I upgraded to a 27 watt model.

Q: 110 v or 220v shows both?

I have one. It just plugged into a receptacle, 110/120v.
