What is the tightener clasp like? Does it have a little button type thing that you press down on to open or is it the type you push in from two sides

Yes, it has a button that you push to slide the mechanism along the cords to make the harness fit tighter or looser.

I have a jrt with a 13' neck and 22" girth around chest. looks like he's right in between small and medium. suggestions?

A medium fits my JRT just fine. It's quite adjustable and a small is a little too snug.

How and where do you measure your dog for this harness?

Neck and chest using a measuring tape. If you don't have one use a piece of string or a belt and then use a ruler to measure the piece of string. Harness is adjustable within a range.
